DENTAL INSURANCE – Guardian Dental

Melmark provides a comprehensive dental benefit for eligible employees and their family members.

VISION

A voluntary vision product that is paid for by the employee is offered through Vision Benefits of America.

EMPLOYEE ASSISTANCE PROGRAM (EAP)

Melmark provides all employees with an employee assistance program. Employees and qualifying family members are eligible for up to eight counseling sessions per family member, per situation, per year. EAP practitioners can assist in a variety of issues, ranging from: marital and family issues, parenting support, health problems, substance abuse, depression, anxiety, stress, work-related problems, grief, financial difficulties, etc. First Call can also assist with childcare, elder care, adoption, college planning and vacation planning.

HEALTH REIMBURSEMENT ACCOUNTS (HRA)

Melmark contributes to a health reimbursement account for benefit eligible employees who are enrolled in Melmark's high deductible health plan. Melmark's contributions cover more than half the deductible and specific contributions vary, based on length of service and coverage level.

FLEXIBLE SPENDING ACCOUNTS (FSA)

Employees may take advantage of an FSA for tax-free medical expenditures and certain dependent-care expenditures through payroll deduction.

SHORT TERM DISABILITY

Melmark's short term disability plan provides 60% of weekly earnings for qualifying disabilities after a 14 day waiting period. This benefit is provided for up to 26 weeks of absence due to a non-work related injury or illness. This benefit is fully paid by Melmark and is provided at no cost to full-time employees who have completed one year of service.

LONG TERM DISABILITY

Melmark's long term disability benefit provides 60% of earnings after a 26-week waiting period for qualifying non-work related disabilities and illness. This benefit is fully paid by Melmark and is provided at no cost to full-time employees who have completed one year of service.

LIFE INSURANCE/ACCIDENTAL DEATH & DISMEMBERMENT

Melmark provides Basic Term Life Insurance and Accidental Death and Dismemberment Insurance to full-time employees. These benefits are provided after the first of the month following the employee's first thirty days of service. Life insurance benefit amounts are equal to one times the employee's annual salary with a minimum benefit of $30,000 and a maximum benefit of $200,000. This benefit is fully paid by Melmark, at no cost to the employee.

PENSION PLAN - TIAA-CREF

Melmark contributes to a defined contribution pension plan for eligible employees. Employees who have completed twelve consecutive months of service and who have worked 1,000 hours or more within a calendar year will receive a pension contribution. Employees must be vested in accordance with the plan description in order to receive any pension benefits.

SUPPLEMENTAL TAX SHELTERED RETIREMENT ANNUITY (403 b)

Tax sheltered annuity plan offers employees the opportunity to save for their retirement on a pre-tax basis. All employees may use the tax shelter annuity program to save for their retirement. Additionally, Melmark will provide a company match for employees who are regularly scheduled to work 30 or more hours a week and who have completed at least one year of service. Melmark will contribute $.40 (forty cents) for each one dollar an employee contributes, up to a maximum of 5% of the employee's total annual earnings. Employees will be vested in Melmark's matching contributions once they have completed three years of service.

TUITION REIMBURSEMENT

Melmark provides a generous tuition reimbursement program to eligible employees. Benefit amount is based on scheduled hours, length of service and course of study. Very attractive reimbursement plan for our ABA certificate program.

TIME OFF BENEFITS

Time off benefits are provided to employees who are regularly scheduled to work 30 or more hours per week. Time off benefits are pro-rated based on scheduled hours for employees working 30 or more, but less than 40 hours per week.

  • Sick / Personal: 7 paid days per year.
  • Holidays: 7 paid days per year.
Vacation Time

Vacation accruals vary based on position and years of service. Vacation benefits are outlined in Melmark's employee handbook.

NOTE: Employees may accrue up to 160 hours of vacation time. Vacation time will stop accruing when there are 160 vacation hours in the time off account. Once vacation time is used and the vacation account drops below 160 hours, accruals will commence. Unused vacation may be carried forward to the next calendar year, provided the maximum account balance of 160 hours is not exceeded. Up to 40 hours of unused sick/personal time may be transferred into an employee's vacation bank at the end of the calendar year provided the maximum vacation account balance of 160 hours is not exceeded.
