This site will guide you through the steps of the pre-employment process. Read the New Employee Checklist below and complete each of the items in the checklist. You can navigate through the site by using the links on the left menu bar. To access many of the tools on this site, you'll need Adobe Acrobat Version 7 or higher.

New Employee Checklist

Step 1: Complete your electronic new hire forms.

You will use an online tool called Onboarding Express to fill out your electronic forms.

Step 2: Print and complete your paper new hire forms.

Bring these completed forms with you to your scheduled Pre-Hire Appointment with HR.

  • FBI Clearance – Dept of Aging
Step 3: Attend your Pre-Hire Appointment with HR.
  • Make sure your electronic new hire forms are completed.
  • Bring completed paper new hire forms with you.
  • Come prepared to have your photo taken for your ID badge.
  • Bring a copy of your highest educational degree received.
  • Bring a copy of your driver’s license and social security card (or birth certificate) or other documents which fulfill your responsibility to provide evidence of ability to work in the U.S.   See the I-9 form for a full list of acceptable documents.
  • Bring a voided check or a form from your bank which provides routing number and account number for direct deposit.  
Step 4: Visit Employee Health.

You will have a confidential post-offer assessment to make sure you are physically ready for work.

Since all job offers are contingent upon the successful completion of a post-offer, pre-hire employee health assessment, it is essential that you complete all responsibilities relative to this health assessment.  Please be sure to bring all of your  completed paperwork with you at the time of the appointment. What to expect at the pre-employment Health assessment.
  1. A drug screening to ensure compliance with Melmark’s drug-free workplace policy.  Please drink an adequate amount of water or liquids as you will be required to provide an urine specimen.
  2. A PPD skin test (also known as TB skin test), blood test and/or chest x-ray.
  3. Industrial Material Handling Test - Dress comfortably for this health assessment.  You will be performing physical activities such as lifting, walking on a treadmill, pushing a wheelchair, etc.  Wear sneakers or athletic shoes.  Do not wear boots, sandals or high heels.
When is My Appointment, and Where Do I Go?
  • Melmark’s Employment Specialist will schedule the pre-employment physical for you.

What do I need to bring?
  • Employee Health forms
  • Your driver's license or other federal or state-issued picture identification.
  • Your immunization records including documentation of prior PPD skin tests, MMR (measles, mumps, rubella vaccine), tetanus status, hepatitis B vaccine history and varicella (chicken pox) history.
  • All prescription medications you have taken in the last 30 days. Please bring the original prescription bottles so that we may verify the medications.
  • A list of all over-the-counter medications you have taken in the past three weeks.
  • Eyeglasses or contact lenses, if you wear them.
Step 5: Print and complete your paper new hire forms

You'll print out these forms in Onboarding Express when you complete Step 1 and bring it with you when you have your ID badge made.

Step 6: Attend orientation

You will attend either a one week or two week orientation program, depending upon your position.  Our orientation programs are designed to familiarize you with the services provided by Melmark as well as with our mission, vision, policies, procedures and work rules. The information provided during Orientation is critical to helping you succeed in your new role.

Orientation is held in Staff Development located on the 3rd floor of our main building and starts promptly at 8:30AM.

During Orientation, please make sure you do the following:

  • Arrive at least a few minutes prior to our 8:30AM start time each day.  You will be expected to be seated and ready to start training promptly at 8:30 AM.
  • Dress casually but appropriately for a professional office environment.   For your safety, open toed shoes or flip flops are not permitted.  Please wear shoes which are flat soled and have adequate traction. All attire is to be clean, in good repair, provide appropriate coverage, and be functional for your position and job duties.
  • Review our Standards of Professionalism (include link here) prior to attending Orientation and be sure to adhere to all Standards of Professionalism during Orientation and throughout your employment with Melmark.
  • Keep your cell phone completely turned off during training hours and work hours.   If you do need to make a call, do so only during scheduled breaks and be sure to be respectful of those around you when talking on your cell phone.
  • Stay alert, attentive and engaged throughout orientation. Come as well rested as possible.
