DENTAL INSURANCE – Guardian Dental
- $50/$150 annual deductible
- Preventive care-100% in network, 60% out of network
- Basic Care-100% in network; 50% out of network
- $2,000 lifetime orthodontia for dependents under 19 years old in network, $1000 out of network
FLEXIBLE SPENDING ACCOUNTS
Employees may take advantage of an FSA for tax-free medical expenditures and certain dependent-care expenditures through payroll deduction.
LIFE INSURANCE/ACCIDENTIAL DEATH & DISMEMBERMENT
Melmark provides active full-time benefit eligible employees coverage for Basic Term Life Insurance and Accidental Death and Dismemberment plans in an amount equal to one times the annual salary to a maximum of $200,000 with a minimum benefit of $30, 000. No cost to the employee.
TIME OFF BENEFITS
Time off benefits are provided to employees who are regularly scheduled to work 30 or more hours per week. Time off benefits are pro-rated based on scheduled hours for employees working 30 or more, but less than 40 hours per week.

*Vacation and Holidays for Special Education teachers are defined by the school calendar.
NOTE:
Employees may accrue up to 160 hours of vacation time. Vacation time will stop accruing when there are 160 vacation hours in the time off account. Once vacation time is used and the vacation account drops below 160 hours, accruals will commence. Unused vacation may be carried forward to the next calendar year, provided the maximum account balance of 160 hours is not exceeded. Up to 40 hours of unused sick/personal time may be transferred into an employee's vacation bank at the end of the calendar year provided the maximum vacation account balance of 160 hours is not exceeded.
TUITION REIMBURSEMENT
Melmark reimburses the cost of tuition up to $3,000 per year for job related college credit courses. Benefit is based on the number of scheduled hours of work per week and length of service. Part-time employees may qualify.
PENSION PLAN - TIAA-CREF
Eligibility Requirements – Completion of twelve consecutive months of employment with Melmark. Contributions are provided to employees who are employed on the last day of the plan year (December 31) and have completed 1,000 hours of service between January 1-December 31. The Melmark Board of Directors determines the amount of the company contribution each year, if any.
SUPPLEMENTAL TAX SHELTERED RETIREMENT ANNUITY (403 b)
Employees may elect to set up a tax-sheltered account to supplement their retirement savings. Melmark will match each dollar that the employee contributes with an additional $.40 (forty cents) to a maximum of 5% of the employee's annual earnings. Supplemental retirement savings are deducted from an employee's pay.
SHORT TERM DISABILITY
Provides 60% of weekly earnings for qualifying disabilities after a 14 day waiting period for up to 26 weeks for absence due to non-work related injury or illness. The benefit is paid for by Melmark and is provided at no cost to the employee.
LONG TERM DISABILITY
Provides 60% of earnings after 26-week waiting period for qualifying disabilities not related to work. This benefit is paid for by Melmark and is provided at no cost to the employee.
